Human Growth Hormone (hGH) is an essential hormone that is associated with sleep, metabolism, mood, energy, concentration, sex drive, and youthful-looking skin.
HGH is a polypeptide hormone made up of 191 amino acids. It is secreted by your pituitary gland, a pea-size gland often referred to as the master gland that sits below your brain at the level of the bridge of your nose. HGH production is controlled by the hypothalamus, part of the base of the brain just above the pituitary gland. The hypothalamus produces two peptides Growth Hormone-Releasing Hormone (GHRH or somatocrinin) and Growth Hormone-inhibiting hormone (GHIH or somatostatin). HGH levels is controlled by these two peptides, which in turn are affected by stimulating factors such as exercise, nutrition, and sleep, and inhibiting factors such as free fatty acids.
The blood levels of human Growth Hormone varies throughout the day, with small and large pulses occurring throughout the day in both women and men. In addition, there is a nighttime enhancement of GH secretion seen during deep (slow-wave) sleep
Secretion of HGH peaks at puberty at about 150 µg/kg day, then drops over 80% to approximately 25 µg/kg/day by age 55. In fact, after age 30, there is a progressive decline of GH secretion by 15% for every decade.
Given HGH's role in muscle strength, fat reduction, cognition and mood, collagen maintainance, metabolism, energy level, and sleep; this begs the question of how much of our reduction of HGH with aging accounts or contributes to the decline in muscle mass, increase in fat, reduced mental acumen and memory, altered mood, poor sleep, fatigue, and skin sagging. Scientific studies unfortunately provide mixed results. One must be aware that association is not necessarily prove cause and effect.
Still, in September 2019, a small and poorly-controlled clinical study in California that was published in the journal Aging Cell suggested fhat it might be possible to reverse the body’s epigenetic clock, which measures a person’s biological age as oppose to the chronological age. For one year, nine healthy subjects took a cocktail of three drugs — growth hormone, plus DHEA and metformin, and reduced 2.5 years of their biological age, as measured by DNA-methylation of the genome using the Horvath clock.. The subjects’ immune systems showed signs of rejuvenation, with increased size of the thymus gland and white-blood cell count, having implication in our body's ability to fight infection and cancer.

HGH secreted by our pituitary gland naturally declines. By age 55, our blood levels of growth hormone are about one-third lower than at ages 18 to 35. This drop coincides with reduced muscle mass and increased body fat.

Adults with human Growth Hormone Deficiency (GHD) comprise two groups. The first are those with a prior diagnosis of GHD in childhood. The second are who acquire GHD as an adult due to hypothalamic-pituitary dysfunction.
Adult-onset GHD's symptoms are subtle and common-place, including fatigue, poor exercise capacity, abdominal obesity, and impaired psychosocial function. Essentially there is no pathognomonic feature. This contrasts with childhood-onset GHD where growth failure acts as a useful biological marker of GHD. Further complicating the presentation, the majority of adults with GHD have deficiencies of other pituitary hormones (ACTH, TSH, LH, FSH, PRL, MSH).
Identifying patients at risk of GHD such as those with hypothalamic pituitary disease, cranial radiotherapy, head injury, and other clinically detectable pituitary hormone abnormalities is important.
IGF-1 is a peptide hormone that mediates most of the biological actions of hGH. Circulating IGF-1 mainly consist of IGF-1 produced in the liver under GH stimulation.
IGF-1 levels less than 2 standard deviation below the age-matched mean is highly suggestive of GHD. Still, IGF-1 can be normal with undisputed GHD, with 37–70% of GH deficient adults having been reported as having normal IGF-1 levels by investigators.
Most scientific studies have shown that patients with adult onset GHD have decreased muscle mass, increased fat mass especially intra-abdominal fat, increased insulin resistance, mild increases in bad cholesterol and triglycerides, reduced left heart mass and function at rest and after exercise, reduced lung function, and reduced bone mineral density; and that these negative changes can be reversed with hGH therapy
But like everything in Medicine, this means appropriate dosage. Ideal therapeutic range is achieved by proper titration to blood levels of IGF-1. Both too little and too much IGF-1 can be deleterious, and can result in increase mortality.
